Overview Registered Nurse Care Coordinator (RNCC) - Stroke Coordinator. Clinician with advanced clinical expertise in acute stroke care, recognized by peers as a leader within the institution. Provides expert care and implements treatment plans in collaboration with patients, their families, physicians, and other members of the health care team. Has aggregate responsibility for nursing care, incorporating current literature and research to guide practice. Identifies strengths within the service area and develops opportunities to mentor others, promote professional growth, and improve patient outcomes. Demonstrates expertise in scholarly activities focused on advancing and promoting the profession.
This role serves as the Stroke Coordinator who is involved in clinical and quality care across the stroke continuum. Activities involve assisting acute stroke patients, rounding with the stroke team, and functioning as a stroke clinical expert and resource. The Stroke Coordinator is responsible for collecting, analyzing, and reporting stroke data and metrics; ensuring compliance with evidence-based guidelines and regulatory requirements; providing staff education, and collaborating on performance improvement initiatives to enhance stroke care quality, patient safety, and outcomes.

Responsibilities
Relationship Based Care: Under the Comprehensive Stroke Center (CSC), collaborates with the interdisciplinary care team to improve stroke patient metrics and outcomes.
Expert Caring: Encompasses clinical assessment, planning, prioritizing, coordinating and implementation of stroke care.
Empowered Leaders: Demonstrates knowledge of and actively participates in shared governance and performance improvement activities related to the CSC.
Lifelong Learners: Demonstrates commitment to ongoing professional growth through formal education, specialty certification, and engagement in internal and external learning opportunities, while recognizing the value of participation in professional organizations. Actively supports onboarding of new team members and provides education on stroke care as appropriate. Maintains membership in an external professional healthcare organization.
Quality Achievement: Adheres to clinical documentation guidelines, understands outcomes data, engages in performance improvement activities and commit to standard work.
Innovation: Integrates evidence-based practice and the application of new knowledge from nursing research and stroke science. Other duties may be assigned.
Qualifications
Education: Bachelor of Science in Nursing from an accredited nursing program.
Experience: 1 year of registered nursing experience required; 3 years of related and relevant RN experience strongly preferred.
Specialty board certification strongly preferred.
License/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.
Physical Demands
Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; attention to detail and ability to write legibly; ability to lift/push/pull 20 - 50 lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.
Compensation : Position Compensation Range: $85,820.80 - $113,401.60 Annual
