Overview The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient- and family-centered care activities between multiple participants, using advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. The RNCC coordinates resources and care activities to support transition management and follow-up across healthcare services and locations.
Responsibilities Demonstrate nursing practice that aligns with the UVA Professional Practice Model through clinical practice, quality achievement, lifelong learning, empowered leadership, innovation, and expert caring.
Plan, coordinate, and prioritize patient care activities considering patients\' unique needs and desired outcomes in collaboration with the inter-professional team, including consult recommendations and escalation as needed.
Maintain safety and continuity of care using documentation, hand-off tools and processes, and related methods.
Collaborate with and advise patients, families, and caregivers in healthcare decisions, respecting culture and values.
Provide health education and coaching tailored to issues identified within treatment and service plans through evidence-based care delivery and safety standards.
Provide facilitative leadership that promotes health equity across the continuum and among interdisciplinary teams to improve population health, patient experience, and cost reduction.
Apply research and evidence-based practices for the improvement of patient care across the lifespan and continuum.
Lead in ensuring continuity and consistency of care across the continuum to facilitate pre-visit coordination, post-clinic follow-up, and handoff between services; monitor and facilitate transitions of care.
Educate patients and families with chronic illness about evidence-based standards of practice to empower self-management.
Identify support needs and develop action plans and guidance to initiate patient-centered care planning and nursing process application.
Contribute to problem-solving through communication and collaboration and evaluate outcomes of treatment options, including tracking progress toward care plans and goals.
Support medication management.

Minimum Requirements Education: Bachelor of Science in Nursing from an accredited nursing program.
Experience: 1 year of registered nursing experience required; 3 years of related and relevant RN experience strongly preferred.
Specialty Board Certification strongly preferred.
License/Certification: Licensed to practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.
Physical Demands Job requires standing for prolonged periods, frequent travel, and bending/stooping. Proficient communication, hearing, and vision; ability to write legibly; ability to lift/push/pull 20-50 lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.
Position Compensation Range $85,820.80 - $113,401.60 Annual
