The Registered Nurse Care Coordinator (RNCC) is responsible for organizing patient- and family-centered care activities across two or more participants. The RNCC uses advanced critical thinking to develop, implement, and evaluate individualized care plans to empower patients and optimize outcomes. The RNCC coordinates resources and patient care activities to support transition management, collaboration across the continuum, and follow-up and coordination of services across healthcare locations.

Care Coordination and Transition Management requires professional assessment, patient risk identification and stratification, and identification of individual patient needs and preferences, including but not limited to the RNCC role.

Responsibilities

Demonstrate the UVA Professional Practice Model through nursing practice, quality achievement, lifelong learning, empowered leadership, innovation, and expert caring.

Plan, coordinate, and prioritize patient care activities in collaboration with the inter-professional team, including consult recommendations and escalation as needed.

Maintain safety and continuity of care using documentation and hand-off processes.

Collaborate with patients, families, and caregivers in healthcare decisions, respecting culture and values.

Provide health education and coaching tailored to treatment plans through evidence-based care delivery and safety standards.

Provide facilitative leadership to promote health equity across the continuum and among interdisciplinary teams.

Apply research and evidence-based practices to improve patient care across lifespan and the continuum.

Ensure continuity and consistency of care across the continuum, including pre-visit coordination, post-clinic follow-up, and handoffs between services; monitor transitions of care.

Educate patients and families with chronic illness about evidence-based standards to empower self-management strategies.

Identify support needs and develop action plans for patient-centered care planning and nursing process application.

Contribute to problem-solving through communication and collaboration; evaluate outcomes and track progress toward care plans and goals.

Support medication management.

Other duties as assigned.

Qualifications

Education: Bachelor of Science in Nursing from an accredited nursing program.

Experience: 1 year of registered nursing experience required; 3 years of related and relevant RN experience strongly preferred.

Specialty Board Certification strongly preferred.

Licensure/Certification: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required.

Position Compensation Range: $85,820.80 - $113,401.60 Annual

PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequent travel, and bending/stooping. Proficient communication, auditory, and visual skills; attention to detail and legibility; ability to lift/push/pull 20 - 50 lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.
