As a Care Coordinator, you will chart each patient’s care trajectory, balance clinical, psycho‑social, financial, and practical needs, and ensure safe transitions from admission through discharge. Your work will be critical to patient outcomes and satisfaction in our Magnet‑designated, teaching environment.
Summary of Essential Duties
Serve as a clinical co‑pilot—charting the best trajectory of each patient’s care by accommodating medical, psycho‑social, financial, and practical needs.
Coordinate the care and service of the patient population from admission through discharge.
Assess, plan, implement, monitor, and evaluate options and services to meet an individual’s health needs and facilitate a safe, timely discharge to the next appropriate level of care.
Qualifications
Current, unrestricted California RN license required.
BSN required.
American Heart Association BLS certification required.
Case Management Certification required within 2 years of hire.
Minimum 2 years of acute‑care nursing (Medical Surgical/Tele/ICU/LTACH) or 1 year in acute‑care nursing (Medical Surgical/Tele/ICU) with at least 2 years of care‑management experience (UM or DC Planning).
At least 3 years of Case Management experience preferred.
Knowledge of medical literature, research methodology, health care delivery systems and financial reimbursement required.
