Responsibilities
Reasons Eating Disorder Center is seeking a Case Manager to manage social work and discharge planning coordination for adult and adolescent patients in our inpatient eating disorder program at BHC Alhambra Hospital.
Key Responsibilities:
Ensure patient attends appropriate level of care/program as indicated on the treatment plan or when status changes.
Maintain necessary documentation of all services and quality of care.
Track and complete utilization reviews, working with third‑party payors regarding certified days.
Contribute to treatment planning.
Coordinate discharge plans to the patient/family/significant others identified by the treatment team.
Provide, monitor, and document discharge planning activities including identification of needs and coordination of referrals to other agencies.
Requirements
Education: RN or Masters/Doctorate in social work, mental health, psychology or related field.
Experience: Minimum 2 years direct clinical experience in psychiatric or mental health setting; preferred 1 year in eating disorders treatment setting.
Additional Skills: Ability to work overtime and flexible hours; Spanish Bilingual preferred; experience in telehealth and electronic medical record documentation.
