We are currently seeking a dedicated and experienced Quality Care Coordinator to join our team and ensure the highest standards of care are consistently met.
Job Summary
The Quality Care Coordinator is responsible for overseeing the quality of care provided by our team of caregivers, ensuring that all services are delivered in accordance with company standards and state regulations. This role involves effective team management, quality control, and ensuring that caregivers perform their duties within the scope of their job requirements. The ideal candidate will have a strong background in home care or a related field, excellent leadership skills, and a passion for maintaining high standards of client care.
Key Responsibilities
Monitor and evaluate the quality of care provided by caregivers, conducting regular visits and assessments to ensure adherence to company standards and client care plans.
Lead, mentor, and manage a team of caregivers, providing guidance and support to ensure they perform their duties effectively and within their job requirements.
Identify training needs and provide ongoing training, education and coaching to caregivers to enhance their skills and performance.
Participate in weekend on-call and after-hours rotations, when needed.
Ensure that all caregiving activities comply with state regulations and company policies. Maintain accurate records of compliance checks and corrective actions.
Serve as a primary point of contact for clients and their families, addressing any concerns or issues related to the quality of care.
Conduct performance evaluations for caregivers, providing constructive feedback and identifying areas for improvement.
Address and resolve any issues related to caregiver performance or client satisfaction promptly and effectively.
Maintain thorough documentation of all quality control activities, caregiver performance records, and client care plans.
Participate in the Quality Assessment and Performance Improvement (QAPI) program.
Qualifications
Minimum of 3-5 years of supervisory experience in home care, healthcare, or a related field, with a focus on quality control and team management.
Bachelor's degree in Healthcare Management, Nursing, or a related field is preferred.
Strong leadership and team management skills, with the ability to motivate and guide caregivers to deliver high-quality care.
Excellent organizational skills and attention to detail, with a focus on maintaining high standards of care.
Exceptional verbal and written communication skills, with the ability to interact effectively with clients, families, and staff.
Strong problem-solving skills, with the ability to address issues and implement solutions quickly.
Proficient in using various apps, computer systems, including care management software and Microsoft Office Suite.
Willingness to travel locally to client homes and work flexible hours as needed.
Ability to work independently with minimal supervision and in a team environment.
