Compensation Compensation Range : $20.99 - $28.34
The referenced pay range represents the minimum and maximum compensation for this job. Individual salaries will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities Provides professional comprehensive care coordination services to patients of the Golisano Children’s Hospital. Collaborates with health, behavioral health and social service providers and is responsible for assessing patient’s needs, enrolling new patients in the Complex Care Program, and coordinating supportive services as needed. Special focus will be serving the most complex, high risk patients enrolled in the Complex Care Program. Services include, but are not limited to: care coordination, heath promotion, comprehensive transitional support, enrollee and family support, referral to community and social supports, use of technology to link services.
Develops, reviews and discusses plans with patient and care team, focusing on linking individuals to Complex Care services, as well as clinical and social services within URMC and outside community providers.
Works in collaboration with Complex Care clinical team, monitors utilization of services and encourages enrollees to follow treatment recommendations, ensures that care is accessible, attended and effective. Conducts routine follow-up, with patients, to ensure patient needs are met.
Interacts with patients via telephonic outreach and utilizes telemedicine capabilities to participate in patient enrollment visits.
Coordinates outreach and engagement activities focused on finding, enrolling and retaining patients in Complex Care Services.

Minimum Education & Experience Bachelor's degree in healthcare related field required or equivalent combination of education and experience
1-year experience in community health, human services, and/or patient liaison/advocate roles preferred
Knowledge, Skills And Abilities Excellent customer service communication skills, ability to prepare presentation materials and present to small groups, computer and organizational skills
