Position Overview
The NARA NW Registered Nurse is responsible for providing comprehensive nursing services to prevent disease and promote health through the application of critical thinking and the ability to exercise independent judgment. The Registered Nurse supports all medical and psychiatric providers in coordinating patient care.

The Psychiatric Nurse Care Coordinator is a registered nurse with clinical experience in medical and psychiatric clients. The nurse care coordinator provides support and guidance to staff as needed and is accountable for patient coordination.

What You Will Do

Work closely with the Triage Nursing Team, monitoring incoming calls and taking lead on all calls related to psychiatry.

Promote clear communication among the care team and treating clinicians, including nursing, prescribing psychiatric professionals, and primary care providers.

Take crisis calls and consult with psychiatric providers and mental health specialists as needed.

Perform chart reviews of clients scheduled to be seen by a psychiatric provider and ensure that the clients are following through with their plan of care.

Coordinate labs, EKGs, appointments, psychiatric medication follow‑up, and assist in the overall management of psychiatric patients, particularly those with chronic diseases or at risk of psychiatric hospitalization, including administration of long‑acting injectables as ordered by prescriber.

Contact patients to report changes in medications or plan of care.

Coordinate prescriptions as needed and assist psychiatric providers with prior authorizations from insurance companies.

Provide education and/or self‑management goal setting as written in the plan of care.

Document all core process work activities in the client’s EHR, including evidence‑based chart reviews, patient calls, correspondence, and patient education activities.

Coordinate continuity of patient care with external healthcare organizations and facilities, including the process of hospital admission and discharge, working closely with the psychiatric mental health intake specialist.
