Overview Psychiatric Case Manager - FT - Day Shift role at Erie County Medical Center Corporation. Involves caring for inpatient or outpatient behavioral health patients, working with a multidisciplinary team to support treatment and crisis intervention as required by program needs.
Responsibilities Perform behavioral health assessments and develop treatment plans using established methodologies.
Act as a primary counselor to individuals, groups, and families; participate in the treatment planning process.
Participate in intake and screening of new patients; collect and maintain data from professional staff and families to guide treatment.
Provide psychosocial histories and counseling to help patients adjust to disabilities and social environments.
Deliver individual treatment and crisis intervention under clinical supervision; support families to accomplish treatment goals.
Observe and document patients' behavioral and psychiatric status and report to the care team; modify treatment plans as needed.
Coordinate discharge planning and outpatient services with community resources; plan treatment and rehabilitation relative to patients’ social situations.
Assist in orientation of new staff and participate in staff development and in-service training.
Participate in quality assurance, peer-review activities, and contribute to team-based care.
Qualifications Possession of a Master’s Degree in Social Work, Counseling, or a Master’s Degree substantially equivalent to Social Work or Counseling as determined by the New York State Education Department.
Special Requirements Possession of a license and current registration as a Licensed Master Social Worker (LMSW) or a Limited Permit to practice as LMSW; or possession of a license and current registration to practice as a Mental Health Counselor or a Limited Permit for Mental Health Counselor with specified licensure timelines.
Possession of Basic Life Support (BLS) Certification from an ECMCC approved provider within 60 days of appointment and maintenance throughout employment.
Additional Special Requirements If assigned to an inpatient care setting, completion of NYS OMH BEWD safety program within three months of appointment and annual retraining.
