PRN Registered Nurse Case Manager

We are currently seeking a Registered Nurse (RN) Case Manager who is passionate about advancing better living and care for seniors. This role covers locations in Galloway, Grove City & Madison County and includes paid mileage reimbursement.

What We’re Looking For

Current and active RN license

1–2 years of home health experience required

OASIS knowledge required

Strong assessment and documentation skills

Dedication to providing exceptional patient care

Essential Functions

Develop, implement, and update individualized plans of care, incorporating physician orders, medications, and input from clients, caregivers, and representatives.

Complete comprehensive client assessments, including physical exams and health histories.

Provide direct patient care, including preventive and rehabilitative procedures, medication administration, and skilled nursing interventions.

Communicate with physicians and physician extenders regarding client conditions, updates, and care needs.

Educate and mentor clients, caregivers, and representatives on disease management, end‑of‑life care, and home care needs.

Collaborate with the interdisciplinary team to ensure coordinated care, discharge planning, and ongoing eligibility evaluations.

Document all care accurately and in compliance with regulatory and organizational standards.

Supervise and evaluate LPNs, LVNs, and Home Health/Hospice aides to ensure competency and adherence to care plans.
