Position Summary:
The PRN Client Assessment Coordinator serves as a field-based leader responsible for conducting client reassessments and care coordination visits on an as-needed basis. This temporary assignment is delegated by Aloma Home Care leadership and does not constitute a permanent change in title or employment status.
This role supports client-centered care through on-site assessment, team communication, and caregiver guidance.
Scope of Responsibilities:
Represent Aloma Home Care in official functions related to client reassessment and care plan coordination
Conduct in-home assessments, reassessments and introductions documenting changes in conditions, environment, or needs
Communicate directly with the client, family members, and assigned care team in a leadership capacity
Provide field-based observations and elevate relevant recommendations to management
Support onboarding, shadowing, or mentorship of other caregivers as assigned
Act as a liaison between Care Managers and field staff for specific cases or transitional plans
Compensation:
$100 flat rate per completed assessment within a 30-mile radius of your residence
For assessments requiring travel beyond 30 miles, you will receive the standard mileage reimbursement rate for the additional distance
Pay is issued bi-weekly and will be reflected in your regular payroll cycle
Terms & Conditions:
This is a PRN (as-needed) role determined by agency discretion and client need
The assignment is temporary and does not represent a permanent change to your job title or employment classification
You remain an employee of Aloma Home Care and must adhere to all policies, procedures, and HIPAA compliance standards
Any misuse of this authority, failure to meet expectations, or breach of confidentiality may result in removal from the role and potential disciplinary action
