Overview
The Hospitals of Providence Rehabilitation Hospital East is a dedicated inpatient rehab facility focused on helping patients regain independence after serious illness, injury, or surgery.

24/7 Availability: Open around the clock to support patients through every stage of recovery.
Comprehensive Rehab Services: Intensive physical, occupational, and speech therapy for individuals recovering from stroke, brain injury, orthopedic trauma, and other complex conditions.
Responsibilities
Coordinate patient care in an interdisciplinary setting.
Support coordination of care, discharge planning, and communication with caregivers and the healthcare team.
Collaborate across disciplines to ensure development and implementation of individualized care plans that meet patient needs and regulatory standards.
Qualifications And Requirements
Education: Bachelor’s or Master’s degree in Social Work or as required by state regulations.
Certifications: Preferred certification in Case Management or Rehabilitation Nursing (e.g., CCM, ACM, RN‑BC, ARN).
Experience: Minimum of 2 years of social work or case management experience in an inpatient setting; experience in acute or rehabilitation hospital preferred.
Skills: Strong verbal and written communication skills in English; proficiency in Microsoft Office applications required.
