HJ Staffing is hiring a compassionate and proactive PORT Case Manager to join our client's Post Overdose Response Team. This critical role supports individuals, families, and communities following a confirmed or suspected overdose event. The PORT Case Manager collaborates closely with a Peer Recovery Specialist to connect individuals to timely, relevant, and culturally informed care using harm reduction strategies where allowable.
This role is perfect for someone passionate about outreach, recovery support, and strengthening community partnerships to improve health outcomes.
Key Responsibilities:
Conduct individual and community outreach visits following overdose events to offer support and connect individuals to services

Serve as a system navigator, helping individuals access behavioral health, medical, housing, and recovery resources

Work as part of a multidisciplinary team, including Peer Recovery Specialists and behavioral health professionals

Build and maintain relationships with diverse community partner s , local providers, and service agencies

Use trauma-informed and harm reduction practices when engaging with participants

Document case activities, outreach efforts, and participant engagement through clear narrative writing

Understand and apply ASAM criteria and follow relevant laws, policies, and procedures in service delivery

Qualifications:
Bachelor’s degree in Social Work, Behavioral Health, Public Health, or related field

Delaware Certified Alcohol and Drug Counselor (CADC) certification or ability to obtain within the first 9 months of hire

Knowledge of ASAM criteria and familiarity with substance use and recovery pathways

Experience with case management, outreach, or recovery support services

Strong communication and documentation skills, including narrative writing

Ability to build rapport and trust with individuals from diverse backgrounds and apply cultural humility in all interactions

Demonstrated understanding of harm reduction strategies (as allowable under the grant)

Preferred Experience:
Leadership or supervisory experience

Experience interpreting and applying public health policies and procedures

Bilingual or multilingual abilities a plus
