PCHH-Chronic Care Coordinator

Overview
Base Wage starting at $16; increased based on experience. Incentive Pay Eligible: Yes

Responsibilities

Assist with referral tracking and feedback.

Assist with performance improvement and data management.

Process enrollment, discharge and transfer forms.

Assist with enabling services such as transportation, food, housing.

Assisting patients with appointment reminders, filling of prescriptions, follow up on self-management and goals.

Skills and Abilities

Ability to communicate clearly and effectively with co-workers and patients.

Ability to complete competency CPR and First Aid.

Ability to prioritize and complete multiple tasks simultaneously.

Effective oral and written communication skills.

Working Conditions
Exposure to distressed patients, families, or visitors. Possible exposure to inside environmental conditions.

Physical Demands
Requires standing, sitting or walking for up to 8 hours or longer per day. The ability to lift up to 50 lbs.

Qualifications

High school graduate or equivalent

Preferred Qualifications

Current Licensed Practical Nurse License in the State of Missouri or Graduate Practical Nurse eligible for examination with valid permit

Certified Nursing Assistant Certificate or certificate from accredited nursing program/college
