Overview Patient Placement Case Manager - Registered Nurse. Full time, rotating shift. Coordinates the placement of all admissions (elective and emergent) and transfers to MMC-NI in accordance with clinical standards and guidelines. Coordinates, prioritizes, and optimizes patient flow throughout the MMC-NI network and facilitates patient entry into MMC-NI from other health care facilities/organizations. Supports the Mission of Trinity Health and Mercy Medical Center-North Iowa.
Responsibilities Coordinate and facilitate the transfer of patients from other healthcare facilities to MMC-NI using Mercy First Call Centers process flows.
Ensure safe and timely placement of patient admissions from all entry points and inter-department transfers using the Patient Placement process flow.
Monitor inpatient bed status and upgrade bed cleaning status via the electronic bed board to expedite patient flow (problem identification/problem solving).
Collaborate with the CNM of Patient Administration and Administrative Coordinator regarding patient placement issues (communication/teamwork).
Alert appropriate personnel to capacity issues (teamwork/problem identification).
Promote safe patient placement according to patient needs, patient placement guidelines, and department scope of service (adherence to policy).
Facilitate decision-making surrounding hospital status changes through ongoing communication with CNM of Patient Administration, Administrative Coordinator, and physicians about census targets and potential A/D/T (communication/teamwork).
Minimum Qualifications Education:
Graduate of an accredited school of nursing
Current RN Licensure in the state of Iowa; BSN or approved BA preferred
Experience:
Five years of clinical nursing experience required; Critical Care or Emergency Room experience desirable
Previous case management experience preferred
Special Skills and Competencies Computer skills with Outlook, ADT system (Healthquest), and related software
Typing ability around 40 words per minute preferred
Ability to read information on a computer screen
Adaptability to multitask and shift focus without loss of efficiency
Flexible hours and workload management
Active participation in personal and professional growth
Excellent problem-solving skills; independent work with sound judgment
High initiative and judgment
Proof of Basic Life Support (BLS Healthcare Provider) current before or within 60 days of hire
Completes Mandatory Reporting: Child and Dependent Adult Abuse within 6 months of hire and renews every 5 years
