Salary based on full-time employment and maximum productivity: $105,000.00 to $115,000.00 per year.

Responsibilities The Home Health Patient Care Manager supervises and coordinates clinical services, providing and directing nursing care to patients in their homes as prescribed by physicians and in compliance with laws, regulations, and policies.
Responsibilities include:
Providing clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team
Managing referrals, clinician assignments, patient orders, and start of care visits
Coordinating home health benefits, medical necessity, and insurance approvals
Assessing patient needs continuously and ensuring individualized, appropriate care that meets eligibility criteria and physician orders
Reviewing assessments and plans of care daily, consulting clinicians as needed
The manager oversees the supervision and coordination of clinical services, ensuring high-quality care in accordance with physician plans and legal requirements, managing an interdisciplinary team.
Additional Responsibilities Receiving referrals and ensuring proper clinician assignments
Coordinating benefits and insurance approvals
Ensuring care is individualized and meets all criteria
Developing and updating patient care plans
Managing communications with physicians, clinicians, and caregivers
Reviewing assessments and plans of care regularly
Coordinating care with all involved disciplines and providers
Following up on tests, physician contacts, and patient condition changes
Facilitating case conferences and reports
Managing orders, documentation, and payer changes
Supervising staff documentation and compliance
Participating in quality and safety programs
Providing direct patient care when necessary
Participating in on-call rotations and other duties as assigned
Education and Experience Current RN licensure in the state of practice
Current CPR certification
Valid Driver's License, vehicle insurance, and access to reliable transportation
