Responsibilities The Home Health Patient Care Manager is responsible for supervising and coordinating clinical services, providing and directing nursing care to patients in their homes as prescribed by physicians and in compliance with applicable laws, regulations, and agency policies.
Provides clinical services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
Receives referrals, ensures appropriate clinician assignments, evaluates patient orders, and schedules start of care visits.
Coordinates determination of patient home health benefits, medical necessity, and insurance approvals.
Continuously assesses patient needs and ensures care is individualized, appropriate, and meets physician orders and eligibility criteria.
Reviews assessments and care plans daily, consulting with clinicians as needed.
Education and Experience Current RN licensure in the state of practice
Current CPR certification
Valid Driver's License, vehicle insurance, and access to reliable transportation
Previous Home Health experience
