Responsibilities
Supervise and coordinate clinical services, directing nursing care to patients in their homes per physician orders and applicable regulations.
Provide clinical services within scope of practice, coordinating with other health‑care team members.
Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plan start‑of‑care visits.
Coordinate determination of patient home‑health benefits, medical necessity, and ongoing insurance approvals.
Continuously assess patient needs and deliver individualized, appropriate care that meets eligibility criteria and physician orders.
Review assessments and plans of care daily, consulting clinicians with recommendations as needed.
Education & Experience
Current RN licensure in state of practice.
Current CPR certification required.
Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
