Key Essential Functions And Accountabilities Of The Job

Assists the care team in coordinating safe, timely, and patient‑centered care.

Assesses self‑management skills of patients and caregivers and encourages wellness through training and community referrals.

Communicates regularly with patients and caregivers regarding the Plan of Care.

Manages internal and external referrals.

Acts as a communication portal between patients and their care team, sharing lab results, generating patient letters, and completing relevant forms.

Performs patient outreach by telephone or text to encourage scheduling with the care team.

Supports care coordination within and outside SEARHC to ensure consistent, effective care delivery.

Facilitates efficient clinic flow and utilization.

Tracks and maintains case‑management and coordination data.

Monitors patient progress and tracks outcomes using SEARHC performance standards.

Advocates for patients and caregivers at the service‑delivery level, empowering decision‑making and self‑care.

Facilitates patient outreach using health and quality data.

Participates in departmental and clinical improvement efforts, maintains a clean and safe environment, and ensures compliance with annual competencies.

Identifies community resources, fosters partnerships, and utilizes resources effectively.

Allocates work: 50 % referral management, 25 % patient outreach, 25 % team communication.

Other duties as assigned.

Education, Certifications, and Licenses Required

Associate’s degree in social work, social services, healthcare administration, business administration, or a related field; or 2 years of relevant professional experience may exchange for a degree.

Bachelor’s degree in a relevant field may substitute for required experience.

Basic Life Support preferred.

Experience Required

Two years of experience in healthcare, behavioral health, dental, or relevant administrative roles.

Internal candidates with at least two years in a front‑ or back‑office role may be considered.

Knowledge of

Available services at SEARHC, tribal health organizations in Alaska, and community resources.

The clinical process and application of this knowledge in practice.

Customer service principles.

Safety and infection control principles.

Skills In

Assessment, anticipation of needs, and data collection.

Use of equipment such as computers and medical devices.

Oral and written communication.

Ability to

Work in teams with providers, nurses, and other healthcare personnel.

Computer Skills

Proficient in Microsoft Office (Word, Excel, PowerPoint).

Other Qualifications

Excellent communication and interpersonal skills.

Strong organizational and time‑management abilities.

Teamwork.

Proficient in EHR systems.

Travel Required

No travel required.

Safety And Risk Management Responsibilities

Comply with safe work rules, report accidents and injuries immediately, cooperate in investigations, and report defective equipment and unsafe conditions.

Physical Demands

Regularly talk or hear; frequently stand, walk, sit, use hands to finger, handle, or feel; reach, lift or move up to 50 lb.

Occasionally climb or balance; stoop, kneel, crouch, or crawl.

Work Environment

Noise level is usually moderate.
