Overview
The WellBe Care Model is a Physician‑Led Advanced Geriatric Care Program focused on the quality of care of frail, poly‑chronic, and elderly Medicare Advantage patients. We bring care to the patient, providing the entire continuum from chronic to urgent care in the home, hospital, skilled nursing facility, assisted living, palliative care, and end‑of‑life care. Our physician‑led teams partner with patients' primary care physicians to deliver concierge level geriatric medical care and social support in the home.
Responsibilities
Our Patient Care Coordinators (PCA) are part of a home‑based care team that provides compassionate care to the frail population who suffer from chronic and acute illnesses or injuries. Responsibilities include:
Answering incoming and outgoing calls and coordinating care with the medical team.
Scheduling patient appointments, specialist appointments, and diagnostic tests.
Serving as a community resource and engaging patients who are currently not engaged.
Maintaining complete and accurate data in patient records.
Employing critical thinking and decision making while using technology.
Working collaboratively with an interdisciplinary team.
Skills & Competencies
Practices the WellBe mission
Updates and maintains digital client records and assists in transitioning hard copy records to digital format.
Acts as a community resource for the patient.
Conducts patient scheduling, registration, and eligibility verification.
Manages patient medical records as appropriate.
Answers phone calls and emails from patients, makes outgoing calls, prioritizes patient concerns.
Answers patient non‑clinical questions and explains the process.
Schedules/coordinates WellBe provider visits with patients, tests, specialist appointments, and social resources.
Ensures all appropriate patient information is in the EMR.
Provides positive customer service and treats all patients and staff with respect.
Prioritizes urgent patient needs and scheduling needs for Complete Care Assessment.
Collaboration and participation in regularly scheduled team meetings.
Supports APC in ongoing panel management and quality of care efforts (e.g., HEDIS).
Completes next‑day visit chart prep.
Utilizes reporting to help providers track, meet, and exceed team goals.
Assists in other tasks needed to accomplish team objectives and goals.
2+ years of experience working for a healthcare practice or hospital.
EMR documentation experience preferred.
High level of professionalism.
Strong critical thinking skills.
Strong customer‑centric focus/service skills.
Strong computer skills, including Word, Excel, and PowerPoint.
Strong verbal, written, presentation, and interpersonal communication skills.

Travel Requirements
Travel may be required up to 25% locally.
Work Conditions
Ability to lift up to 20lbs. Lifting or transferring of patients may involve lifting up to 50lbs and assisting with weights of more than 100lbs. Ability to stand for extended periods and drive to patient locations (home, hospital, SNF, etc). Requires fine motor skills and visual acuity.
