From time to time, support the patients in the rehabilitation service, and Older Persons Mental Health.***The role is 3 days per week (22.8 hours) – one day must be a Wednesday.*****What we can offer you**We care about your wellbeing at HammondCare and as a result offer a wide range of health and lifestyle benefits including:* NFP salary packaging ($15,900 tax free) plus meals and entertainment benefit (up to $2,650 tax free)* Additional leave purchase options and 14 weeks paid parental leave pro-rata* Flexible working options* Discounts on everyday products and services* Discounted healthcare memberships - access to network of fitness centres and pools for employees and families* Free access to independent counselling support for employees and families* Health & Wellbeing programmes* Reward & Recognition programmes* Referral bonuses* Career and professional development opportunities**Responsibilities*** Provide pastoral care and support to patients, relevant families and staff of HammondCare* Be the point of contact for relevant internal and external support/referral services* Act as a role model for staff and as an advocate for HammondCare’s Mission, Motivation and Mission in Action, and its Christian values* Share pastoral expertise and coach staff to support the spiritual needs of patients* Participate in multidisciplinary team meetings and make a contribution about the patient’s emotional/spiritual needs and goals and contribute to the development and ongoing evaluation of a care plan* Facilitate regular carer support groups* Network with other religious leaders and services and coordinate relevant care, resources and group activities* Strategically develop tailored resources, ideas and activities to enhance the spiritual and emotional wellbeing of patients and families, collaborating with management, staff and volunteers* Communicate and build trust by developing relationships with staff and volunteers.* Liaise with managers, case managers and attend staff and case management meetings as required* Training staff on spiritual care and wellbeing* Leading memorials, ANZAC, and special services* Leading and training a team of pastoral care volunteers* Working and collaborating in a Pastoral Care team**About you*** Strong understanding and experience of people living with life limiting illness, the impact on carers, and journeys of grief and bereavement* Strong personal and active Christian faith and church involvement* Theological /Pastoral Care qualifications and experience* Palliative care experience - desired* Demonstrated capacity to pastorally care for and enhance the wellbeing of those in need* Highly developed communication skills including listening, compassion and empathy* Experience in team environments* Strong administrative and computer skills**Our Culture**HammondCare is committed to creating an inclusive workplace that promotes and values diversity and inclusion.
