Located on Nantucket island. The Palliative Care Nurse Specialist is responsible for providing direct patient care and clinical management for patients and families. The Palliative Care Nurse works collaboratively with the Nurse Practitioner Director of PASCON to provide comprehensive palliative care for patients and families. The Palliative Care Nurse Specialist collaborates with physicians, Mid-level providers, nurses, social workers, and other appropriate staff within the hospital organization, as well as with outside agency staff to ensure team-based, coordinated, comprehensive care and seamless transitions for patients in any setting (hospital, home, nursing home).

Valid Drivers License and vehicle for visits required. Experience Nursing 2-3 years required Hospice and palliative nursing care 2-3 years preferred Knowledge, Skills and Abilities - Strong clinical skills. - Effective collaboration skill ability with all levels. - Ability to rapidly make accurate decisions, analyze, and interpret information. - Strong communication and interpersonal skills with demonstrated ability to teach and develop others.
