Role Overview
The Advanced Illness Management (AIM) Palliative Care RN will report directly to the Director of Palliative Care. You will work with patients, their families, and other health‑care professionals to provide and coordinate high‑quality end‑of‑life care. AIM Palliative RNs offer in‑person and telephonic assessments to palliative care patients in collaboration with the interdisciplinary team and provide education and comfort in home environments.

Responsibilities

Conduct in‑person/telephonic RN assessments and provide education to palliative care patients, family members or caregivers.

Complete Risk Stratification Assessment to identify patient support needs per established protocols.

Observe patient visit frequency per individual need and risk identification.

Provide reliable education and instructions to patients and families with consistently excellent customer service.

Consult with and educate patients/families regarding disease process, self‑care techniques, end‑of‑life care, medications, nutrition, and dietary needs.

Communicate findings that identify appropriate preventive and rehabilitative skilled nursing or therapy needs to PCP or AIM Palliative NP.

Educate patient and family on medication administration and treatments as prescribed.

Coordinate care with the palliative team members.

Communicate change in condition or signs of decline to Palliative NP and/or PCP.

Support hospice referral as appropriate.

Document timely clinical records on all patients including screening findings, orders, progress notes and care plans.

Support care plan development and implementation from initial assessment through discharge.

Collaborate on development and modifications to individualized palliative plan of care, in consultation with patient and family.

Facilitate obtaining care when need is identified per Plan of Care.

Participate in evening/weekend call as required.

Facilitate prescription transmission to pharmacy, including controlled substances as delegated.

Receive laboratory results, provide them to the nurse practitioner in a timely manner, and place durable medical equipment requests as medically necessary.

Communicate clinical findings to NP or PCP as needed, and reassess effectiveness of care plan regularly.

Maintain effective working relationships with supportive care team.

Establish and maintain working relationships with all contacted in course of work.

Document and communicate clinical findings and treatment in record consistent with standards.

About You
Registered Nurse / RN Requirements
Education

Registered Nurse

Experience

2 years of prior skilled RN experience.

Demonstrated ability to provide quality patient care.

Experience in palliative care, oncology, hospice, or geriatric care preferred.

Additional preferred experience: hospital staff RN, Med‑Surg, PACU, float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing.

Licenses, Certifications And/or Registration

Active RN license to practice in the state without any board action.

CPR Certification.

Equipment / Tools / Work‑Aids

Fluent competence in Microsoft Outlook, Word, and Excel.

Fluent in EMR documentation.

Specialized Knowledge And Skills

Prior clinical experience.

Strong compassionate phone skills.

Independence and sound critical decision‑making.

Competent to perform after‑hours in‑person visits.

Familiar with needs of advanced illness patients and families.

Excellent written and verbal communication.

Maintains patient confidentiality.

Good problem‑solving skills.

Good time‑management skills.
