Overview
Lee Health is seeking a Palliative Care Physician!

Lee Physician Group Palliative Care is seeking a physician with fellowship training and BE/BC in Hospice and Palliative Care Medicine to join their busy team. Currently, the team consists of 4 physicians and 13 APRNs. This exclusive physician group is team-oriented and quality-focused, and is currently seeking physicians interested in providing a mix of both inpatient and outpatient care. Candidates for this position will be bedded interested in doing coordinated care, medication & chronic symptom management, end-of-life management, and goals of care discussions.

Our outpatient care is being redesigned for full integration into some of our high-utilization clinics (specifically oncology and advanced heart failure). This is a great opportunity for someone with an interest in development, program growth and out-of-the-box thinking with a high focus on exceptional patient care to join the team and assist with this strategic redesign process.

Responsibilities

Inpatient and outpatient care – inpatient care will be provided at the hospital setting with potential SNF opportunities as well

Weekly call rotation is by phone 1:4-1:6
