Overview Lee Physician Group Palliative Care is seeking a physician with fellowship training and BE/BC in Hospice and Palliative Care Medicine to join their busy team. The team currently consists of 4 physicians and 13 APRNs. This exclusive physician group is team-oriented and quality-focused, and is seeking physicians interested in providing a mix of both inpatient and outpatient care. Candidates will be interested in coordinated care, medication and chronic symptom management, end-of-life management, and goals of care discussions.
Position Information Inpatient and outpatient care - inpatient care will be provided at the hospital setting with potential SNF opportunities as well
Weekly call rotation is by phone 1:4-1:6
Responsibilities Provide a mix of inpatient and outpatient palliative care services
Conduct coordinated care, medication and chronic symptom management
Manage end-of-life care and goals of care discussions
Participate in program development and redesign efforts for outpatient integration
Collaborate with a multidisciplinary team of physicians and APRNs
