Salary Range: $150,000 to $155,000 per year.

Responsibilities The Palliative Care Nurse Practitioner/APRN provides direct patient and family care through palliative care consultation to ensure specialty services to patients with serious illnesses and a prognosis of one year or less who are not ready or eligible for hospice care.
Demonstrates effective and ethical use of approved evidence-based palliative medicine practices in delivering all patient care based on physical, emotional, spiritual, psychosocial, ethical, educational, safety, and related criteria, appropriate to the ages of patients served.
Incorporates palliative care body of knowledge to formulate an effective plan of care.
Gathers pertinent information systematically from all sources, including medical records, family history, caregivers, and thorough physical, emotional, psychosocial, and spiritual assessments.
Completes comprehensive assessments utilizing palliative care domains, interprets findings, conducts medication reconciliation, formulates diagnoses, initiates plans of care with therapeutic interventions, and evaluates outcomes.

Education and Experience RN/APRN licensure in the state of practice
Nurse Practitioner/CNS with prescriptive authority, maintaining all licensure requirements
At least 5 years of clinical nursing experience, including 1 year as APRN with Hospice, Palliative Care, or Oncology experience preferred
Advanced practice palliative care certification (ACHPN) preferred; certification in an appropriate advanced practice area will be considered. BCLS licensure required. ACHPN certification encouraged after 2 years of employment or when requirements are met.
