Position Summary
Join Providence at Home with Compassus as a full‑time Palliative Care Nurse Practitioner. The program was launched in 2021 and currently serves 85 adult patients. The NP will conduct home visits (in‑person) with oversight from the medical director and program director, coordinating with PCPs, specialists, and a multidisciplinary team. This role requires prescribing medications, including controlled substances, and managing a minimum patient census of 80 patients.
Responsibilities
Perform routine and urgent assessments, triage consults by patient acuity, and provide bedside palliative care with patient/family education.
Order, perform, and interpret laboratory and radiology tests within scope of practice.
Prescribe medications, including controlled substances, to the extent delegated and licensed.
Consult with palliative care physician or designees as needed; inform primary physician and collaborate with other physicians.
Assist in all facets of care coordination for palliative care referrals.
Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement.
Comply with applicable laws, regulations, and collaborative agreements.
Complete accurate billing and coding activities for all patient encounters according to CMS guidelines.
Provide training, ongoing education, and onboarding support for staff.
Define goals for professional growth and actively participate in professional activities and organizations.
Engage in self‑care activities for personal and professional growth and longevity.
Adhere to confidentiality regarding patients, families, staff, and the organization.
Assist patients and families in identifying goals of treatment and ongoing plans of care; communicate these goals among the interdisciplinary team (IDT).
Collaborate and communicate with the medical director, attending physicians, hospital staff, and IDT; participate in IDT meetings; collaborate on clinical and psycho‑social interventions; assist with other IDT member interventions; effectively communicate patient and family needs to IDT; submit required clinical documentation within 24 hours of visit completion.
Educate patient and caregiver regarding care, disease process, goal setting, symptom control, treatment options, prognosis, and advance care planning.
Supervise nurse trainees, including graduate and advanced practice nurses in field placements; participate in supervision of other trainees, including medical students, residents, fellows, social work, and psychology trainees.
Require regular regional travel.
Perform other duties as assigned.
Qualifications
Master's Degree in Nursing.
Minimum of 2 years in an acute or transitional care setting.
Active and unencumbered RN license in the state of practice.
National Provider BLS – American Heart Association (Vendor Managed) upon hire.
Education and Experience
Master's Degree Nursing; 2 years acute or transitional care experience.
Certifications, Licenses, and Registrations
Active RN license.
National Provider BLS – American Heart Association (Vendor Managed).
Physical Demands and Work Environment
Regular standing, walking, manual dexterity, lifting up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and focus adjustment. Exposure to bodily fluids, infectious diseases, and standard medical equipment usage.
Compensation and Benefits
Compensation range: $56.93 – $73.67 per hour.
Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
