About the Role:

The Palliative Care Home Visit Nurse Practitioner plays a critical role in delivering compassionate, patient-centered care to individuals with serious, chronic, or life-limiting illnesses in their homes. This position focuses on managing symptoms, improving quality of life, and supporting patients and their families through complex medical and emotional challenges. The Nurse Practitioner will collaborate closely with interdisciplinary teams to develop and implement personalized care plans that address physical, psychological, and spiritual needs. By providing expert clinical assessment, medication management, and education, the role ensures that patients receive holistic care tailored to their unique circumstances. Ultimately, this position aims to enhance patient comfort and dignity while facilitating effective communication between patients, families, and healthcare providers.

Minimum Qualifications:

Current and unrestricted Nurse Practitioner license in the state of Georgia.

Master’s or Doctoral degree in Nursing from an accredited program.

Certification as a Nurse Practitioner with a focus on adult, geriatric, or family practice.

Minimum of 2 years clinical experience in palliative care, hospice, or related field.

Basic Life Support (BLS) certification.

Preferred Qualifications:

Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.

Experience conducting home visits and working within community-based healthcare settings.

Strong knowledge of pain and symptom management protocols specific to palliative care.

Familiarity with electronic health record (EHR) systems and telehealth technologies.

Training in communication skills for difficult conversations and end-of-life care.

Responsibilities:

Conduct comprehensive home visits to assess patients’ physical and emotional health status and needs.

Develop, implement, and adjust individualized palliative care plans in collaboration with patients, families, and interdisciplinary teams.

Manage and monitor symptom control, including pain management and other distressing symptoms, using evidence-based practices.

Provide education and counseling to patients and their families regarding disease progression, treatment options, and advance care planning.

Coordinate care with physicians, social workers, chaplains, and other healthcare professionals to ensure seamless service delivery.

Document all patient interactions, assessments, and care plans accurately and timely in compliance with regulatory standards.

Respond promptly to patient and family concerns, providing crisis intervention and emotional support as needed.

Participate in quality improvement initiatives and continuing education to maintain best practices in palliative care.

The Nurse Practitioner will utilize advanced clinical assessment skills daily to evaluate complex patient conditions and tailor care plans accordingly. Effective communication skills are essential for educating patients and families, facilitating shared decision-making, and coordinating with interdisciplinary teams. Strong organizational abilities support managing multiple home visits, documentation, and follow-up care efficiently. Empathy and emotional intelligence are critical in providing compassionate support during sensitive end-of-life discussions and crisis situations.
