The Palliative Care and Hospice Coordinator coordinates patient care within the Hospice and Palliative Care program, including outpatient clinic, consult service, inpatient hospice unit, and outpatient hospice. Responsibilities include attending care management rounds and interdisciplinary meetings, assessing patient eligibility and conducting concurrent care reviews, understanding Hospice and Palliative Care policies and regulations, working towards and/or maintaining accreditation through Joint Commission certification, ensuring service quality, effectively using the Epic system, and maintaining knowledge of relevant Medicare and Medicaid regulations, state licensing regulations, Conditions of Participation for hospice, Joint Commission and state standards, Quality Standards, QAPI, Medicare Compliance, and standards of practice for hospice and palliative care.

Position Details:

Minimum Qualifications:
Education: Associate's degree in nursing.
Licensure: Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements.

Certification(s): Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire. 

Skills, Knowledge Abilities:
•    Proficient in conducting patient assessments and developing care plans.
•    Strong understanding of pain management and symptom control techniques.
•    Familiarity with Medicare and Medicaid regulations, state licensing regulations, and Joint Commission standards.
•    Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
•    Strong organizational and time-management skills to handle multiple tasks and prioritize effectively.
•    Ability to work collaboratively in an interdisciplinary team environment.
•    Proficiency in using electronic health records (EHR) systems, Epic preferred.

Preferred Qualifications:
Education: Bachelor of Science in Nursing (BSN).
Experience: Three years' experience in a clinical setting, preferably working in acute care, long term care, home health, hospice, or palliative care. 

Certification(s): HPNA Certification in Hospice and Palliative Care.

Skills, Knowledge Abilities:
•    In-depth knowledge of Hospice and Palliative Care practices, policies, and regulations
•    Understanding of Quality Standards, Quality Assessment and Process Improvement (QAPI), and ethical billing practices.
•    Ability to perform clinical procedures relevant to palliative and hospice care.

Physical Requirements: 
•    Position requires the ability to push, pull, and/or lift 50 lbs. on a regular basis. 
•    Position requires prolonged standing and walking during each shift.
•    Position requires the ability to grip, reach, bend, kneel, twist, and squat to perform duties.
