Position Summary
Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff.
Essential Duties And Responsibilities
Deliver high-quality palliative care to patients with medical necessity for palliative service via in-person and telehealth options.
Assist with design of operational processes associated with development of a palliative care program.
Work with facilities and Eventus employees to ensure the growth of the palliative care program.
Train and educate internal employees on palliative care principles and processes.
Recognize gaps in processes and work with interdepartmental team members to streamline/organize palliative operations.
Qualifications/Education
Master’s degree in Nursing from an accredited institution and a current, valid license to practice nursing issued by the state in which you reside. All other certifications necessary to perform duties must be current and valid.
OR
Graduated from a physician assistant educational program accredited by a National Commission on Accreditation of Allied Education Programs.
Passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
Licensed by the state in which you reside as a Physician Assistant.
Specialization in geriatric, family, or psychiatry desired.
