POSITION: OUT OF HOME CARE COORDINATOR (Part-Time)
CLASSIFICATION CODE: NON-EXEMPT, PART-TIME
DEPARTMENT: OUT OF HOME
REPORTS TO: OUT OF HOME PROGRAM MANAGER
STATEMENT OF JOB: This position provides care coordination services to clients enrolled or eligible for enrollment with White Mountain Apache Behavioral Health Services Tribal Regional Behavioral Health Authority during office hours, overnight, weekends, and holidays, as required. The Care Coordinator acts as a link between clients, their support systems, community, and healthcare/social service professionals, primarily focusing on acute stabilization services. This role also advocates for clients with other service providers as appropriate.
DUTIES AND RESPONSIBILITIES:
Coordinate acute stabilization services for clients identified through behavioral health assessments under the medical direction of Indian Health Services staff.
Facilitate referral processes with psychiatric facilities, transportation providers, and Indian Health Services staff to ensure continuity of care.
Document contact with clients to assess needs, maintain required paperwork, and engage family members in accordance with AHCCCS and ABHS standards.
Adhere to all applicable policies, procedures, and program standards of ABHS, White Mountain Apache Tribe, Indian Health Services, AHCCCS, and other relevant authorities.
Facilitate access to healthcare services and provide informal counseling, education, and support following AHCCCS guidelines for mental health, demonstrating flexibility and competence in handling clients with traumatic experiences and complex psychological needs.
Address diverse spiritual, emotional, and social needs in various environments using a trauma-informed care approach while maintaining professionalism and cultural awareness.
Engage clients and their families in appropriate healthcare services.
Verify and print Medifax confirmation information for intake as needed.
Complete data entry for new clients into the electronic health record.
Work effectively with a wide range of clients, including adults and families.
Collaborate with court/probation, tribal/community agencies, families, and natural support systems to meet client needs and develop strong community partnerships.
Provide services at clinics, in the community, shelters, residential facilities, hospitals, and client homes to ensure access to benefits and services.
Participate in multidisciplinary meetings to advocate for clients and coordinate services.
Maintain client records following HIPAA, AHCCCS, ABHS, Federal, State, Tribal, and local regulations.
Uphold ethical policies and guidelines as per state laws, rules, policies, and discipline-specific ethics codes.
Serve as a positive role model through leadership, self-discipline, and genuine care, treating all clients and staff with dignity and respect.
Participate in all required ABHS training sessions.
Report to work timely and communicate directly with supervisors regarding attendance.
Perform other duties as assigned by the supervisor.
MINIMUM QUALIFICATIONS:
High school diploma or equivalent with at least four years of relevant experience in mental health, case management, substance abuse, or related fields; a bachelor’s degree is preferred.
Ability to pass pre-employment testing, including fingerprint clearance and background checks.
One year of experience working with Native Americans is preferred.
Excellent communication skills; proficiency in Apache language is a plus.
Critical thinking, problem-solving, organizational, and time management skills; ability to work independently with flexibility.
Strong interpersonal skills to work effectively with diverse individuals and teams.
Proficiency in MS Office and operation of office equipment.
Valid Arizona Driver’s License and reliable transportation.
NOTE: Preference is given to qualified Indian candidates per the Indian Preference Act (Title 25, U.S. Code, Sections 472 and 473).
ESSENTIAL FUNCTIONS:
The physical demands include constant standing and frequent use of hands, walking, bending, squatting, climbing, kneeling, twisting, grasping, reaching, and lifting objects up to 25 pounds. Management reserves the right to reassign duties as needed. Employment is at-will, terminable by either party at any time without cause or notice.
