Job Summary: The Violence Prevention Specialist/Care Coordinator (Nurse, MSW, LSW, LCSW) will serve several roles for the Comprehensive Hospital-Based Adolescent Violence Assessment and Treatment at Children's Hospital Colorado (CHAVAT@CHCO). This is a full-time (1.0 FTE) grant-funded position.
Key Responsibilities: Coordinate patients interfacing with the CHAVAT, including reviewing charts for eligibility for the program, providing outreach to families, and coordinating intakes.
Conduct assessments and provide brief motivational interviewing interventions for youth at risk for or already perpetrating violence.
Conduct MI-focused interventions focusing on ways to reduce violence perpetration, gun carrying, and gun violence in the outpatient setting.
Communicate treatment plans to behavioral health specialists involved in the CHAVAT and coordinate appointments with youth and families.
Coordinate wrap-around services, setting up outpatient management appointments for youth identified in emergency or hospital settings.
Participate in the care of approximately 100 clients annually.
Perform other activities as deemed necessary by the supervisor.
Work Location: Onsite - this role is expected to work onsite in Aurora, CO.

Qualifications: Minimum Qualifications: Bachelor's degree in social work or a related field
At least 2 years of clinical experience working with adolescents and families
Licensed Social Worker (LSW)
Applicants must meet minimum qualifications at hire.
Conditions of Employment: Pass a drug test through Children's Hospital Colorado

Preferred Qualifications: Master's degree in Social Work (MSW)
Registered Nurse (RN) license
Licensed Clinical Social Worker (LCSW)
At least 3 years working with at-risk adolescents and young adults
Proficiency in Motivational Interviewing and trauma-informed care
Knowledge, Skills, and Abilities: Counseling skills
Organizational and basic computer skills
Excellent communication skills
Ability to collaborate with healthcare teams
