Overview OB RN Care Manager, Highland Women\u2019s Health role at Highland Hospital of Rochester NY. Under general direction of the Highland Women\u2019s Health (HWH) Nurse Manager and Division Director, the OB RN Care Manager\u2019s central role integrates and coordinates access and utilization management, proactive patient management, care facilitation and treatment planning for the obstetric patient population throughout the HWH practice. The RN Care Manager facilitates clinical aspects of a patient centered medical home, working with the interdisciplinary medical team in the provision of patient care. May also participate in supervision of other clinical staff members.
Responsibilities Coordinate access and utilization management for obstetric patients within the HWH practice.
Provide proactive patient management and assist in treatment planning.
Facilitate care delivery within a patient-centered medical home model in collaboration with the interdisciplinary team.
Support supervision of other clinical staff members as applicable.
Qualifications Fulfilled the educational requirements to be a licensed RN in New York State. BSN preferred.
Experience: Three to five years of professional nursing experience. Outpatient OB/GYN nursing experience preferred. Progressive clinical experience; community health, care management, and/or disease management preferred.
Experience working as a member of a multidisciplinary team is required. Supervisory experience desirable. Data analysis experience preferred. A satisfactory equivalent of education and experience will be considered.
Licenses/ Certifications: Registered nursing license (New York). Fetal monitoring certification (NCC or PQF) current or to be obtained within 6 months of hire. Current CPR certification (AHA BLS for health care providers or ARC CPR for professional health care providers). Successful completion of medication proficiency examination. Valid New York State driver\u2019s license and ability to travel.
Details Salary Range: $77,220-$96,300 Annually
