Job Description: Nurse Practitioner Specialization: Palliative Care

Location: Jackson, Pike, Scioto, and Gallia counties

We are seeking a Palliative Care Nurse to share our vision of advancing better living and care for seniors!
ESSENTIAL FUNCTIONS Conduct physical exams and comprehensive medical and social histories, including healthy individuals and those with acute or chronic conditions. Make independent decisions regarding management and treatment.
Order, perform, and interpret laboratory tests for screening and diagnosis.
Prescribe and recommend appropriate diets, medications, and treatments such as physical therapy.
Perform therapeutic or corrective measures, including urgent care.
Refer patients to specialists, nurses, physicians, or other healthcare providers as appropriate.
Provide education, counseling on health maintenance, promotion, social issues, prevention, management, and medication use.
Evaluate the effectiveness of interventions and provide additional support as needed. Follow up with noncompliant patients.
Serve as a preceptor for students in medical, nursing, NP, or PA programs.
Consult with other healthcare providers regarding patient care in various settings.
Communicate with insurers, employers, government agencies, and other providers about patient care as needed.
Manage patient care plans, monitor progress, determine need for referrals, hospital admissions, or emergency care, and resolve illnesses.
Assess social and economic factors affecting each patient and tailor care accordingly.
Familiarize with community resources and connect patients with appropriate services.
Establish contracts with patients regarding responsibilities.
Maintain productivity levels to meet facility goals.
