As a Nurse Practitioner, You Will
Work in collaboration with the Palliative Care Medical Director and other team members to deliver diagnostic, treatment, and follow‑up care for patients referred to palliative care.
Take responsibility for patient palliative care from initial assessment through discharge, covering disease‑specific changes, functional decline, nutrition and cognition, pain or other distressing symptoms, falls, infections, disease burden, and family quality of life.
Establish individualized plans of care with patients, families, and the Palliative Care Team.
Facilitate care ordering and obtain care as identified in the plan.
Participate in evening/weekend call as required, providing clinically competent and responsive care.
Prescribe medications, including controlled substances, and order laboratory, diagnostic tests, treatments, and durable medical equipment within scope.
Communicate findings to the Palliative Medical Director no less than bi‑weekly and reassess care plans regularly.
Maintain effective relationships with the supportive care team (e.g., social worker, spiritual care coordinator) and involve team members when warranted.
Document clinical findings, care plans, and interventions in the medical record consistently with acceptable standards and reimbursement requirements.
Show competency in Palliative Medicine quality‑measure collection, documentation, and reporting.
About You – Requirements
Excellent leadership, interpersonal, written, and verbal communication skills.
Independent decision making and self‑motivation.
Knowledge of principles of palliative medicine, Medicare/Medicaid regulations, and interdisciplinary processes.
Minimum of Masters of Science in Nursing (or equivalent) and two years of Nurse Practitioner experience; specialty in family or adult medicine preferred.
Experience with Part B billing and autonomous practice in an advanced practice role.
Licensure: Current RN license, current Nurse Practitioner certification, collaborating physician agreement or Palliative Medical Director agreement, Part B provider certification, federal and state DEA license, ACHPN preferred.
Ability to work efficiently with point‑of‑care software.
