The Palliative Care Nurse Practitioner facilitates palliative care for Gentiva patients, ensuring a comprehensive, individualized treatment plan while demonstrating excellent clinical skills and ethical judgment. The role provides diagnosis, treatment, consultation, and follow‑up under the direction of the Palliative Medical Director (if required under state law).

Responsibilities

Collaborate with the Palliative Care Medical Director and other team members to deliver palliative care.

Take responsibility for patient palliative care from initial assessment through discharge, assessing disease‑specific changes, decline, nutrition, cognition, pain, falls, infections, disease burden impact, and caregiver/family quality of life.

Establish an individualized palliative care plan in consultation with the patient, family, and team.

Facilitate care access when needed in accordance with the plan of care.

Participate in evening/weekend calls as required, acting in a clinically competent and responsive manner.

Prescribe medications, including controlled substances, and order laboratory and diagnostic tests, treatments, and durable medical equipment within scope.

Communicate clinical findings to the Palliative Medical Director at least bi‑weekly and reassess care plan effectiveness, modifying as needed.

Maintain effective relationships with the supportive care team (Social Worker, Spiritual Care Coordinator, etc.) and involve them when indicated.

Document clinical findings, treatment plan, and care provided in the patient’s medical record consistently with acceptable standards.

Know and apply Palliative Medicine quality measures, documentation, and reporting.

Qualifications & Experience

Minimum of a Master of Science in Nursing from an accredited school, or equivalent under state law.

Minimum of two years of Nurse Practitioner experience; preference for Family or Adult Medicine specialty.

Experience with Part B billing and ability to work autonomously in an advanced practice role.

Licensure as a registered nurse and Nurse Practitioner in the state.

Current Part B provider, DEA (Federal and State if required), and, preferred, ACHPN certification.

Collaborative agreement with a supervising physician or Palliative Medical Director in accordance with state law.

Strong leadership, interpersonal, written, and verbal communication skills; independent decision‑making and self‑motivation.

Working knowledge of palliative medicine principles, Medicare/Medicaid regulations, interdisciplinary process, and home health or hospice care pathways.

Proficiency with point‑of‑care software and compliance with continuing education requirements.

Valid driver’s license and automobile insurance if required.
