As a Nurse Practitioner, You Will
Work in collaboration with the Palliative Care Medical Director and other team members to provide diagnosis, treatment and follow‑up for patients referred to palliative care.
Take responsibility for patient palliative care from the initial assessment through discharge. Assessment should include disease‑specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, impact of disease burden, caregiver and family quality of life.
Establish an individualized palliative plan of care, consulting with the patient, family and other team members.
Facilitate obtaining care when a need is identified in accordance with the Plan of Care.
Participate in evening/weekend call as required, providing responsive on‑call service.
Prescribe medications including controlled substances within scope, order and interpret laboratory and diagnostic tests, order treatments and durable medical equipment as indicated.
Communicate clinical findings to the Palliative Medical Director at least bi‑weekly, reassessing and modifying the care plan as needed.
Maintain effective relationships with supportive care team members and involve them when appropriate.
Document clinical findings, treatment plan and care in the patient’s medical record in a manner that supports sound medical practice and reimbursement.
Be knowledgeable of Palliative Medicine quality measures, documentation, and reporting.
Qualifications
Nurse Practitioner Requirements
Minimum of a Master of Science in Nursing from an accredited school of nursing (or equivalent under state law); minimum of two years of Nurse Practitioner experience; Nurse Practitioner specialty in Family or Adult Medicine preferred; practice experience with Part B billing; ability to work autonomously in an Advanced Practice Role.
Specialized Knowledge/Skills
Excellent leadership, interpersonal, written and verbal communication skills.
Independent decision‑making and self‑motivation.
Working knowledge of principles of Palliative Medicine, Medicare/Medicaid regulations, and interdisciplinary processes.
Meet mandatory continuing education requirements of Gentiva and licensure board.
Efficient use of point‑of‑care software.
Licenses & Certifications
Current RN license or equivalent required for Nurse Practitioner licensure.
Current Nurse Practitioner certification/license.
Collaborative agreement with a supervising physician or the Palliative Medical Director as required by state law.
Part B Provider certification.
Federal DEA license.
State DEA license (if required).
ACHPN – Certification in Hospice and Palliative Care (preferred).
Valid driver’s license.
