Overview Our Inpatient palliative care team serves patients 7 days a week. After your orientation period, your schedule will be determined based on need and discussed at the time of the interview. There is no on call duty associated with this role.
Schedule Monday through Friday, 8:00 AM to 5:00 PM.
Caseloads and Visits Variable caseloads based on schedule and patient census.
Consultation visits up to 90 minutes; follow‑up visits 15‑45 minutes.
Productivity goal: 6‑8 patients daily, 1‑2 new consults with 5‑6 follow‑up visits.
What you’ll be doing Provide inpatient care and quality care across diverse socioeconomic and cultural backgrounds.
Educate caregivers on disease education, advance care planning, medication administration, and end‑of‑life expectations.
Receive a company phone and other equipment to perform duties.
Coordinate care with community medical providers and other disciplines, placing new orders and updating during and between visits.
Participate in an interdisciplinary team (IDT) comprising a physician, nurse practitioner, registered nurse, and medical social worker. The team meets each morning at 8:30 AM to review patient assignments and discuss complex cases as needed; a monthly business meeting is held with the CMO.
Collaborate with inpatient hospice clinical liaisons to coordinate and facilitate patient care at end‑of‑life.
