Nurse Practitioner, Inpatient Palliative Care, (Greenbrae), Full-Time, Days
MarinHealth is hiring for the Nurse Practitioner - Palliative Care role at MarinHealth Medical Center in Greenbrae. This position collaborates with the healthcare team to develop and implement interdisciplinary care plans for patients with complex health needs and/or poorly managed medical conditions. The PC Nurse Practitioner will help patients navigate the inpatient healthcare system, support symptom management, and coordinate care within the Marin County health and social services community.

Job Description Summary
The Nurse Practitioner - Palliative Care leads the development and implementation of an interdisciplinary care plan for patients with complex health needs, in collaboration with the healthcare team. The role focuses on patient- and family-centered care, symptom management, and coordination of inpatient services with community resources. The position is based at MarinHealth Medical Center.
Conducts initial in-person and/or virtual assessments for patients referred for palliative care services, evaluating goals, beliefs, caregiver support, living status, social services, and behavioral health status.
In partnership with the patient, caregivers, and treating providers, participates in developing and implementing a patient-centered, individualized care plan aligned with the patient’s goals.
Assists patients in navigating the system of providers and social service agencies; supports symptom management and reassessment of goals.
Educates patients and families on care plans and medication management and coordinates care with internal and external teams.
Interfaces with interdisciplinary teams (primary/specialty care, spiritual care, social work, pharmacy) and represents the program positively to internal and external customers.
Participates in department meetings, quality improvement, and training initiatives.
Contributes to public health programs and acts as a liaison to hospitals, long-term care, home health, and community agencies.
Completes required documentation promptly and adheres to department and facility policies.
Education
Graduate of an accredited school of nursing; BSN preferred.
Completion of an accredited Nurse Practitioner Program with MSN degree.
License/Certifications
Current RN license in California and successful completion from an accredited NP program.
Active and unrestricted NP license in California.
Current NP Furnishing License (if applicable) and CMS payer enrollment eligibility.
National board certification (required by first day of work) and DEA license to California as needed by specialty.
BLS Certification (American Heart Association).
Experience
Three or more years of experience in an acute care setting and/or community-based home health or hospice care.
Bilingual skills strongly preferred; broad clinical background and palliative care experience preferred.
Knowledge of motivational interviewing and Medi-Cal regulations preferred.
