Overview Nurse Practitioner - In Home & Virtual Chronic Care with a $10,000 sign-on bonus. Base pay range: $120,000.00/yr - $145,000.00/yr. This role supports Accompany Health’s mission to deliver dignified, high-quality care to low-income patients with complex needs, through at-home and virtual care and 24/7 support.

About the role Accompany Health Advanced Practice Clinicians serve a key role in integrated care teams that include Physicians, Community Health Workers, Patient Experience Navigators, RNs, Social Workers, Behavioral Health Clinicians, Psychiatrists, and Pharmacists. The team provides and coordinates holistic, patient-centered care for patients with complex medical, behavioral health, and social needs.
As an Advanced Practice Clinician, you will provide longitudinal care and build trusting, therapeutic relationships with patients. You may see patients independently or with physicians in the home, community, and virtually. You will ensure comprehensive chronic disease management, preventative care, wellness, and urgent care as needed. Smaller panel sizes with strong multidisciplinary support enable more time with patients and a greater impact on health outcomes.

Responsibilities Providing exceptional patient-centered, holistic primary care via home or community-based visits and virtual visits
Developing and leading comprehensive care plans for your panel through participation in multidisciplinary case conferences
Collaborating with external providers and community organizations to advocate for patient-centered care aligned with patient goals
Performing annual wellness visits and comprehensive patient assessments with attention to biopsychosocial factors and thorough documentation
Providing clinical coverage as needed, including home-based follow-up and urgent visits
Participating in rotating call schedules to address urgent clinical needs
Ensuring timely and accurate documentation within the EMR and other care platforms
Mentoring and training fellow advanced practice clinicians

Desired skills and experience Required
Active Nurse Practitioner license in the state of Michigan
AANC or AANP board certification for Nurse Practitioners
Active DEA Controlled Substance Registration or eligibility; X-waiver for buprenorphine is a plus but not required
Active state controlled substance license in the employment state or eligibility for application
Certified in Basic Life Support for Healthcare Providers
2+ years of clinical experience in primary care (internal medicine, family medicine, or geriatrics)
Ability to perform independent clinical assessment and decision-making for acute and non-acute problems
Comfort with electronic medical records
Valid unrestricted driver’s license and access to a insured vehicle for daily use
Preferred
Experience in adult internal medicine, family medicine, geriatrics, addiction medicine, and palliative care
Experience providing home-based care
Experience caring for individuals with complex chronic medical, behavioral, and social needs
Experience engaging individuals with untreated or symptomatic chronic mental illness and addiction
Prior experience in a multidisciplinary care team
Experience in value-based care organizations

Compensation and benefits The base salary range for this full-time position is $120,000-$145,000 plus bonus, equity, and benefits. Salary ranges are determined by role, location, and experience. The precise salary for your location will be discussed with our talent team during the hiring process.
