Registered Nurse – Case Manager
We are seeking a Registered Nurse to service our patients throughout the Myrtle Beach area. The role of the Nurse Case Manager is critical to the patient’s care team. The nurse will visit patients and facilitate collaboration with providers, traveling to patients’ homes, long‑term care facilities, and other sites to evaluate patients and initiate telemedicine visits.

Responsibilities

Facilitate receiving all medical records from primary provider and specialists.

Review medical records, complete consents, and enroll patients in Care Management as eligible.

Initiate a Care Management Plan of Care.

Capture all diagnoses at highest specificity.

Complete AWVs, cognitive impairment screenings, SDoH assessments, ACPs.

Evaluate for home health, hospice, palliative care, or consults with specialty division.

Evaluate for RPM devices, resources, or tools that may improve quality of life.

Communicate and coordinate care, reconcile prescribed medications, supplements, herbal remedies, and other treatments.

Provide post‑discharge education, evaluate for adaptive equipment, DME, safe environment.

Documentation and logging of Care Management activities; maintain up‑to‑date care plans.

Qualifications

Licensed RN, license in good standing.

Minimum of 3 years clinical experience preferred.

Experience in community settings preferred.

Proven ability to communicate and collaborate with interdisciplinary care teams, patients, and caregivers.

Strong written and verbal skills.

Basic computer knowledge.

Leadership skills, teamwork, and judgment.

Flexible hours, travel between facilities; licensed driver with reliable transportation.

Other Information
Full‑time, salary‑based 8‑hour shifts Monday‑Friday (8 am‑5 pm). Must be able to travel daily to service area 29577/29572.
