Overview Nurse Case Manager I - Adult ED role at MUSC Health .
The Nurse Case Manager (NCM) I reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the NCM I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The NCM I serves as a clinical expert related to discharge planning, resource utilization, and case management.

Responsibilities

Coordinate a plan of care to achieve clinical outcomes and promote safe, timely discharge.

Manage the plan of care across the care continuum to ensure efficient movement of the patient through the hospital system.

Serve as a clinical expert related to discharge planning, resource utilization, and case management.

Minimum Training and Education

Bachelor’s degree in Nursing from an accredited school of nursing and three years of work experience as a Registered Nurse required.

Prior leadership experience preferred. One year of case management and/or utilization review work experience preferred. Case Management certification preferred.

Required Licensure, Certifications, Registrations

Licensure as a registered nurse by the state of South Carolina or compact state required.

Current Basic Life Support (BLS) or ACLS required (AHA BLS forHealthcare Providers or equivalent, or American Red Cross CPR/ AED for Professional Rescuer and Healthcare Provider).

Additional Job Details

Hours per week: 40. Schedule: Days, Monday-Friday, occasional weekends & holidays. No nights.
