Overview Nurse Case Manager - Hospice Registered Nurse
We are seeking a Registered Nurse to service patients in the Aiken/North Augusta area. The Nurse Case Manager is a critical part of the patient’s care team, visiting patients, coordinating with providers, and initiating telemedicine visits. Travel to patients’ homes and facilities (including ALFs and ILFs) is required to evaluate patients and coordinate care in accordance with applicable laws, regulations, and policies within the scope of practice. This is a full-time, salary-based role with 12-hour or 8-hour shifts.

What you’ll do (Responsibilities) Facilitate receiving all medical records from the patient’s primary provider and specialists.
Review medical records and complete patient consents.
Enroll eligible patients in Care Management and initiate a Care Management Plan of Care.
Capture diagnoses at the highest specificity and resolve gaps; ensure they are accepted.
Complete AWVs and cognitive impairment screenings; perform Social Determinants of Health (SDoH) assessments and/or screenings.
Complete Advanced Care Planning (ACPs) to be reviewed with the patient by the provider.
Evaluate home health, hospice, palliative, or consults with Your Health Specialty Division; assess RPM devices or tools to improve quality of life.
Communicate and coordinate care; reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
Provide post-discharge education and evaluate for adaptive equipment and DME; assess safe home environments.
Evaluate acute conditions or follow-up needs; document and log Care Management activities with the care team to keep plans updated.
Coordinate with the patient’s health care team, including providers, therapists, home health or hospice representatives, and other care plan participants.
Facilitate visits with appropriate providers or entities; initiate telehealth visits when necessary.
Coordinate with the patient’s hospice interdisciplinary team and participate in IDG meetings when needed.
What we’re looking for (Qualifications) Must be a Registered Nurse with a license in good standing.
Minimum of three (3) years of clinical experience preferred; experience in community settings is a plus.
Proven ability to communicate and collaborate with interdisciplinary care teams, patients, and caregivers; strong written and verbal communication skills.
Basic computer knowledge; ability to manage multiple responsibilities and demonstrate sound judgment.
Excellent interpersonal skills and ability to foster a cooperative work environment; team player who can travel between offices and facilities.
Must have reliable transportation and a licensed automobile insured per state/organizational requirements.
