Join us as a Nurse Case Manager in Fresno, CA, covering the surrounding areas. This field‑based role requires home‑based work with travel up to 2 hours one‑way (4 hours round trip per appointment) and may involve multiple appointments in a day. You will manage work‑related injury cases, coordinating care and communication among insurance carriers, medical providers, attorneys, employers, and employees, ensuring progress is monitored and results reported back to stakeholders.
Responsibilities
This role includes the full spectrum of medical case management for injured/disabled individuals: assessment, planning, coordination, implementation, and evaluation of care.
Qualifications
Current, unencumbered Registered Nurse (RN) license in California
Experience in case management of workers' compensation cases, preferred
Certified Case Manager Certification (CCM) strongly preferred; must be obtained within two years of hire date
Strong organizational, timeline adherence, PC and technology, and written/ verbal communication skills
Ability to operate autonomously with minimal oversight
Skilled at patient education
Valid driver’s license and good driving record
