Nurse Care Manager (Remote) - Faculty Practice Plan
Position Summary
Under general supervision, the Nurse Care Manager will support population health and quality initiatives with a focus on preventative health care measures, aiming to improve health outcomes through telephonic and written communication. Primary duties include performing telephonic patient outreach to schedule appointments, obtaining health records from external providers, and sending reminders to patients to complete chronic care gaps as identified by the Care Manager. The role is administrative‑focused, with the majority of patient interaction occurring via telephonic outreach.
Scheduled Hours
40 hours per week.
Primary Duties & Responsibilities
Integrates evidence-based clinical guidelines and protocols to develop patient‑centric treatment plans that promote quality and efficiency in the medical home setting.
Assesses, plans, implements, coordinates, monitors, documents, and evaluates options and services to optimize member health status.
Develops systems of care that monitor member progress and promote early intervention in acute care situations.
Establishes collaborative relationships with providers, payers, and other health team members to optimize interventions.
Manages utilization and practice metrics to refine care delivery and maximize clinical, quality, and fiscal outcomes.
Completes required paperwork and documentation in a timely manner.
If electronic medical record order entry is part of the job, completes requisitions/orders per WashU guidelines.
Provides proactive care outreach to patients overdue for preventive services to support value‑based care initiatives; collaborates with providers, clinical staff, social work, case management, and community resources to remove care barriers.
Develops and implements patient‑centered strategies to improve chronic condition management, clinical outcomes, and patient satisfaction.
Performs special projects and other duties as assigned.
Working Conditions & Physical Demands
Normal office environment; exposure to blood‑borne pathogens.
Typical duties include sitting at a desk, standing, walking, bending, and occasional lifting (>25 lb).
Requires protective devices; office and clinical equipment used.
Education
B.S.N. – Bachelor of Science in Nursing.
Required Qualifications
Registered Nurse in the state of Missouri or Illinois.
Basic Life Support certification (online BLS without skills assessment does not qualify).
License to practice nursing in Missouri or Illinois; license valid and in good standing.
Knowledge of Epic EHR and other EMR systems.
Certifications
Basic Life Support – American Heart Association or American Red Cross.
Registered Nurse – Illinois Department of Financial and Professional Regulation or Missouri Division of Professional Registration.
Work Experience
No specific work experience is required; however, experience in clinical patient care, ambulatory care, and electronic medical records is preferred.
