Overview Nurse Care Coordinator (Poison Center) – Phoenix, AZ
Posting Number req24153. Department: Poison Control Center. Location: Greater Phoenix Area, Phoenix, AZ USA. This position supports the Arizona Reach program, a 24/7 service that coordinates acute medical care transfers, facilitates placement calls, connects practitioners, and follows placement progress to help referring facilities focus on patient care. The role emphasizes addressing healthcare disparities and advancing equity in care, ensuring efficient and seamless discharge planning and continuum of care for AZ REACH patients.

Duties & Responsibilities Foster collaborative relationships with care teams across the healthcare continuum, including physicians and payers.
Collect and disseminate relevant, timely information to the patient\'s healthcare team to ensure comprehensive care and to meet utilization and regulatory requirements.
Assist care teams in connecting patients to appropriate post-discharge services and coordinate specialty referrals with outpatient care teams.
Maintain knowledge of Medicare, Medicaid, PRC, and other benefit programs to support discharge planning and ongoing care.
Stay informed about community resources available in the patient\'s area to address their specific needs and access to care.
Monitor and ensure comprehensive documentation of the patient\'s care plan, progress, and all communications with involved parties.
Provide supervision and training to other staff members as needed and perform assigned duties to support department needs.
Provide on-call support during scheduled rotations or as required.

Minimum Qualifications Bachelor\'s degree or equivalent advanced learning attained through professional level experience required.
Minimum of 5 years of relevant work experience, or equivalent combination of education and work experience.
Preferred Qualifications Bachelor\'s degree in Nursing strongly preferred.
Case Management experience preferred, or a minimum of 3 years of relevant work experience, or an equivalent combination of education and work experience.
