KenCrest is seeking a Nurse Care Coordinator (RN) to provide comprehensive care coordination and management services to an assigned caseload of supported individuals. This role focuses on optimizing health outcomes, improving quality of life, and reducing healthcare costs by ensuring timely and appropriate care. The Nurse Care Coordinator acts as a liaison between supported individuals, families, healthcare providers, and community resources.
Responsibilities include coordinating and managing treatment, care, appointments, and follow-up, completing health assessments, documenting nursing notes in the EMR system, managing hospitalizations, conducting biweekly home visits, and providing education and training to direct care providers to ensure continuity of care.
Qualifications:
Valid RN license in Connecticut
DDS nursing orientation upon hire or within 6 months
CPR (BLS) certification
Preferred: 3 years’ nursing experience with individuals with developmental disabilities
Basic computer skills (Outlook, Microsoft Office)
Valid driver’s license
Clear criminal and background checks
