Description We are currently seeking a compassionate and dedicated Nurse Practitioner (NP) specializing in Hospice and Palliative Care. In this role, you will play a crucial part in providing quality end-of-life care to patients, ensuring their comfort and dignity during challenging times. Responsibilities: Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
Develop and implement individualized care plans in collaboration with the interdisciplinary team.
Manage symptoms such as pain, nausea, and shortness of breath through appropriate interventions.
Provide education and support to patients and their families on end-of-life care options.
Coordinate care with other healthcare providers to ensure holistic patient care.
Document patient care activities accurately and timely in electronic health records.
Participate in regular team meetings to discuss patient progress and care plans.
Adhere to all regulatory standards and guidelines related to hospice and palliative care.
Requirements: Active Nurse Practitioner (NP) license in the state of practice.
Certification in Hospice and Palliative Care preferred.
Strong communication and interpersonal skills.
Ability to provide compassionate and empathetic care to patients and families.
Knowledge of pain and symptom management in palliative care.
Experience working in end-of-life care or hospice settings is a plus.
Proficiency in electronic health record systems.
Ability to work collaboratively in a multidisciplinary team environment.
About the Position: Schedule: Monday-Friday (8a-5p) or 4x 10-hour shifts
No call
Setting: Outpatient
Daily Census: 10-20 patients
Will travel to different homes, facilities, and clinics
APPs practice independently with some supervision
EMR: AthenaHealth
Board certification required
Preferred Experience: 2+ years
