Summary Provide supportive services to households who are facing an impending eviction or condemnation. Coordinate direct assistance programs to ensure data entry accuracy and timely reports.

Works in cooperation with HouseCalls staff to provide housing and health assistance to high-risk families and individuals.
Coordinate the rent assistance program to ensure compliance with grant requirements, update forms, attend grantee trainings and meetings, manage and lead data quality, collection, entry and reporting requirements in HMIS.
Join team in answering office phone and responding to voicemails.
Communicates case updates to co-workers both verbally and in writing to ensure complete documentation for all clients.
Researches and identifies new resources to meet client needs and communicates information to co-workers.
Documents on social service activities according to program and grant requirements in an accurate and timely manner.
Make referrals as needed for the cases and calls that are received.
Assess and plan strategies to meet identified needs by applying knowledge and skills and understanding of funding rules.
Participate in staff meetings, quality improvement activities and community meetings as requested.

Supervisory Responsibilities None. May assist in training and orienting new staff members or interns.
Work Environment This role requires regular walking. This role may come into contact with patients who may have contagious illnesses. A hybrid work-from-home schedule, 3 days per week, is optional.
Physical Demands Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift 15 pounds at times.
Travel Requirements Must have the ability to travel between MCC sites in the St. Paul/Minneapolis metro area.
Required Education And Experience Bachelor’s degree in social/behavioral science or equivalent.
Four years’ experience in case management or similar roles (social work, public health, etc.)
Preferred Experience Familiarity with MAXIS (Ramsey County) software
Previous use of HMIS-Client track
Data Management
