The Non-Medical Case Manager will work closely with clients living with HIV/AIDS who have multiple psychosocial and/or health-related needs. Non-Medical Case Management services adopt a strength-based approach, including treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, legal services, social support, and other needed services.
The goals of Non-Medical Case Management are to ensure that clients with HIV/AIDS have timely access to comprehensive medical care and social services; prevent disease transmission; delay HIV progression; and promote client independence and self-sufficiency. The Non-Medical Case Manager will be supervised by the Medical Case Manager Supervisor. Prior experience working with the HIV/AIDS community is preferred.
Responsibilities include: Collaborating with Medical Providers in the ID clinic to identify clients in need of Medical Case Management at intake.
Completing patient screening to determine eligibility for medical case management services.
Consulting with Clinical Staff to identify current clients requiring medical case management services.
Responding within 24 hours to requests for case management services.
Providing feedback to the patient’s care team in the ID clinic as needed.
Determining client eligibility for various RW funded services, including MCM, Virginia ADAP, and other community resources.
Providing orientation to new clients about clinic operations.
Obtaining proper documentation for proof of HIV status, residency, income, and insurance status for initial and recertification eligibility determinations.
Conducting client intake interviews and completing all required forms.
Referring patients to MCM to schedule assessment appointments.
Performing six-month eligibility reviews and recertification, obtaining necessary documents.
Maintaining documentation and program notes per Virginia Department of Health standards.
Completing client data entry into VDH’s & HRSA databases.
Coordinating with outreach and care staff to facilitate access or re-engagement in care.
Coordinating eligibility and intake services with community agencies and healthcare providers.
Maintaining current information on community resources for client support.
Engaging face-to-face with clients at least every 90 days; more frequent visits if required for ongoing case management.
Documenting treatment adherence activities, including appointment keeping, medication adherence, and pharmacy issues.
Performing other duties as assigned.
Minimum Requirements: High School Diploma
1 year experience working with patients/clients living with HIV or 2 years of related experience.
Physical Demands: Primarily sedentary, involving extensive computer use; occasional travel required.
Preferred Qualifications: Experience with the HIV/AIDS community is preferred.
No licensure required.
